Job description

Who We Are

At Entyre Care, we serve the elderly and adults with disabilities who want to stay in their homes with trusted caregivers. We serve thousands of people across the country, and we're launching a certified personal care agency in Wisconsin and looking for the RN who will build it with us.

The Opportunity

You are the clinical leader for Entyre Care Wisconsin – you will set the standard of care, build and lead a team of RNs, establish and maintain our core clinical operating procedures, and ensure every client receives high-quality, compliant services.

What You'll Do

  1. Build, lead, and manage a team of RN Supervisors — hire, train, set expectations, and hold them accountable for clinical quality across the service area

  1. Establish clinical standards and workflows: how assessments get done, how care plans are written, how supervisory visits are conducted, and how your team documents everything

  1. Oversee all client care delivery — ensure assessments, care plans, physician orders, and 60-day supervisory home visits are completed accurately and on schedule

  1. Conduct direct client assessments and supervisory visits yourself, especially during early growth

  1. Train and evaluate personal care workers on the specific tasks they’re assigned, and ensure your RN Supervisors do the same as the team scales

  1. Own compliance — quality assurance, infection control, EVV, documentation standards, prior authorization tracking, and survey readiness

  1. Represent Entyre Care to regulators, physicians, and partners

What You'll Bring

  1. Active, unrestricted Wisconsin RN license

  1. At least 1 year of supervisory or administrative experience in personal care, home health, or a related program

  1. Experience with patient assessments, care plans, physician orders, and direct care staff training

  1. Comfort managing other RNs — setting clinical standards, reviewing their work, and coaching them

  1. Reliable transportation for home visits across the service area

Strong pluses: experience as Administrator of Record or DON at a PCA or home health agency; familiarity with DHS 105.17 and DHS 107.112; experience with Medicaid billing or EVV systems; BSN or higher.

You don’t need to check every box. If you’ve supervised direct care staff and want to build and lead a clinical program, we want to talk.
