This role frequent travel. You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).

Job requirements

What You’ll Bring

  • Must hold an active Registered Nurse (RN) License in the state of employment

  • Minimum two (2) years of experience in direct care of elders or adults with disabilities

  • Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required

  • Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting

  • Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc)

  • Experience with Minimum Data Set (MDS) assessment tool a plus.

  • Assess client status upon initial home visit to identify needs in order to develop a comprehensive case management plan.

  • Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting nursing and related needs.

  • Document in an accurate and timely manner. Maintain a clinical record for each morbidity patient.

  • Completing the MDS-HC and other nursing assessments while coordinating all other applicable clinical assessments.

  • Training, evaluating, and supervising caregivers.

  • Monitoring the health status of all members to ensure that all needed care is properly delivered.

  • Proficiency in common software and web-based applications including Microsoft Outlook, Excel, Word, PowerPoint, and Teams.

  • Additional state-specific qualifications may be required.

Key Responsibilities

  • Conduct home visits to assess and create personalized care plans.

  • Train and support caregivers, ensuring they can meet their loved ones' needs.

  • Monitor and report on clients’ health, coordinating with their primary care physicians.

  • Facilitate smooth transitions and ongoing care for clients.

  • Maintain compliance with all relevant regulations and standards.

  • Full-time hybrid position offering autonomy while utilizing your nursing assessment, care planning and case management skills
